How to Apply for NSO Marriage Certificate

Any person who changes his/her civil status (from single to married) is required to update his/her record with the civil registry by applying an NSO Marriage Certificate.  Moreover, a change in status will also require an updating of one’s personal data which is necessary in applying for a passport, SSS or GSIS benefits, birth certificate of one’s child/children, PRC license, driver’s license, and other official documents issued by the government. Private companies also require their employees to update their personal data especially their civil status should they get married before they can avail of any benefits accorded by the company. Changing one’s status will necessarily result in the change of names of women, thus, it is imperative for them to submit a Marriage Certificate.

 

Here is the list of important requirements in applying for NSO Marriage Certificate:

Application for Marriage Certificate

One valid ID

  • Current employees ID with the signature of the employer or authorized representative;
  • Postal ID;
  • Students ID or library card, signed by the school authority;
  • Senior Citizens ID;
  • Driver’s license;
  • NBI/PNP clearance;
  • Passport;
  • SSS/GSIS ID;
  • Integrated Bar of the Philippine (IBP) ID;
  • License issued by the Professional Regulatory Commission (PRC)

NSO Marriage Certificate

You need to provide the following information when filling up the application form:

  • Number of copies being requested;
  • Name of husband;
  • Maiden name of wife;
  • Place of marriage;
  • Date of marriage;
  • Date of registration, if the marriage certificate was registered late; and
  • Purpose of your request
  • For representatives: relationship to the owner of the Marriage certificate
  • Fee: P330 per copy

Application for Certificate of NO Marriage

  • One valid ID (the same as the list above)

Application for Certificate of No Marriage

You need to provide the following information when filling up the application form:

  • Name
  • Gender
  • Place of birth
  • Date of birth
  • Mother’s maiden name
  • Father’s name
  • Purpose of your request
  • Fee: P430 per copy

Application for Certificate of No Marriage

After filling up the application forms, you may submit your application to any nearest National Statistics Office (NSO) branch or through the NSO website for online application.
